          Format: 85 slides on PowerPoint illustrating my development through
          crucial stages of childhood, adolescence, young adulthood
          Time: 40 minutes

PICTURE THE STRUGGLE FOR EMPOWERMENT:
Audience -young adults in transition and their parents; employers; vocational service providers and trainees; self-advocacy groups
Format: 80 slides on Power Point illustrating preparation for employment, barriers to successful employment in the community and
the rewards of productivity; contrast with supported self-employment
and its opportunities for a special population
Time: 45 minutes

CHOOSE LIFE, CHOOSE CHANGE:
Audience  – vocational and residential service provider staff; consumers in independent living; family support teams
          Format: 90 slides on PowerPoint illustrating life before and after an
          extreme change in lifestyle; learning along with staff; changes in staff
          goals and attitudes; rewards of healthy eating and increased activity;
          budgeting, menu building and shopping tips.
          Time: 50 minutes
          Note: This presentation is the introduction to Dreams Come True,
          the 10-week program on fitness and health described below.

DREAMS COME TRUE:
Supported Lifestyle Change
“Healthy eating and energetic activities make DREAMS COME TRUE.”
Class members: people with disabilities and their support people, either staff or family member. Sponsors: provider agencies. Location: provided by sponsor.
          Format: ten 90 minute classes with audio and visual materials,
          Game playing, food preparation demonstrations, team approach

DREAMS COME TRUE offers to change past practices in the development of health and wellness among consumers. In the past we may not have paid due attention to the importance of consumer choice, small working teams, communication between support people and consumers, adaptations to the needs of individual learners, follow-up strategies and the leadership of trainers who have themselves successfully walked the walk.

DREAMS COME TRUE tackles issues of health and wellness with a comprehensive focus on the many factors that affect healthy eating and energetic activity as well as the inter-relationships among a variety of strategies designed to reach both the consumer and his or her support team in order to achieve the following outcomes.

  • Establishment of a team approach to healthy eating and energetic activity
  • Support staff and family members familiar with current, basic principles of healthy nutrition
  • Acquisition of practical skills by consumers, skills adapted to each one’s specific need and level of interest
  • On or more leaders among participating consumers and support people who have the tools to carry the program forward
  • Follow-up strategies planned with the agency’s administrators who might also be observers at program sessions
